Understand the Process
Before you begin this application, please review our Refund, Scholarship & Waiver Policies for this program. Parent-signed waivers are required for any youth’s participation in any Activate Good volunteer activity or Program without direct parent/guardian supervision. You may return a scanned copy of your teen’s waiver by email; all waivers must be signed by the teen’s legal parent/guardian. Deadlines will be communicated based on application date.
In order to be eligible, applying teens must be rising freshmen, sophomores, juniors, or rising seniors in high school at the time of the Program (June and July 2019). Teens from previous program years (2017 or 2018) are welcome to return for our 2019 Program.
Now, let’s go over the information you’ll need in order to complete this application, and the steps that will come after submitting this application:
YOU WILL NEED:
- To review instructions with your teen
There are 2 applications for this program: A Parent application, and a Teen application. Your teen will be asked to create a video, no longer than 3 minutes, about themselves, their interests, and their idea for a project to benefit the community. Please ensure your teen is clear on instructions for their application and video. Program applications will be considered complete only when both Parent and Teen sections are submitted.
- Ample Time
We estimate that this application could take somewhere between 15-45 minutes to complete, depending on the time it takes your teen to create their video. You cannot save as you go, so you may want to save your answers elsewhere.
- At least one (1) parent/guardian and the applying teen present to complete this application
Your teen should create their video and complete their application independently as this section is intended to help us get to know them better. You can review the Teen application video criteria here before you get started.
- The applying teen’s medical information and dietary restrictions
Insurance information, dietary restrictions, medications, and any other applicable medical information is requested solely for the purpose of ensuring your teen’s safety throughout the Program. Dietary/allergy information will inform our catering and snack provisions.
- Contact information for at least one (1) emergency contact
This contact may also be your alternate transportation/pickup person. Please be sure the phone number you include in this application is a direct line, preferably a cell phone number.
- Household Income (Optional)
This information is only required for anyone interested in receiving a NEED-BASED scholarship, should one become available; if you do not supply this information, your teen will not be considered for a need-based scholarship. Scholarships are not guaranteed, may be awarded in half or full amounts, and are based on funding availability, the time-stamp on each submitted application, the merit of each teen’s responses in this application, and demonstrated need by the teen’s parent/guardian. Further information may be requested later, if applicable. If accepted to the Program, you may still be responsible for a deposit of $50, which may be refunded after the teen’s Program week is complete this summer. Please review all policies here.
AFTER COMPLETING THIS APPLICATION:
- Parent/Guardian #1 should receive emails containing all submitted information
When completing the application, please be sure that the most responsive parent/guardian is listed in the “Parent/Guardian 1” prompt. Keep the email confirmation in a safe place for your record.
- This information will also be sent to an administrator for review
You will be contacted by that administrator to communicate Program approval, fees, further required paperwork, or any other applicable information. (Due to a high volume of emails, please wait to be contacted. Thanks in advance for your patience and cooperation.)
- If approved, your teen will receive an approval letter and be assigned to a Program week, and you’ll receive a link to pay your Program fee of $300
This letter will be a PDF sent to both the teen and Parent(s) via the emails provided in the application. Your assigned week will be based on the week you select in your application; please be sure you retain availability for the week(s) you select until you hear from us about your approval and week assignment. Full payment is due immediately at acceptance, unless an alternate date is communicated or an immediate scholarship is awarded.
